Before admission to any Hurlbut Care Community, all residents will need to provide a PRI (Patient Review Instrument), NYS screening, medical history and physical, advanced directives and financial information. Other items may be needed depending on a resident’s diagnosis.
If transferring from a hospital, simply let the social worker know you are interested in one of our Hurlbut Care Communities. The social worker will obtain all of the information and fax it to the Hurlbut Care Community of your choice.
If coming to us from home, a PRI and screen will need to be completed, and information obtained from their physician in the community.
The Patient Review Instrument (PRI) is a medical evaluation tool that determines whether or not an individual is eligible for skilled nursing home placement, and it is mandated by law that a PRI be performed by a registered nurse. A PRI and Screen is valid in New York state for 90 days as long as the person’s health status does not change.
We are utilizing Senior’s Choice Care Management and Together in Caring to provide the preparation of Patient Review Instruments and Screens for individuals needing skilled nursing level of care coming from home, or other facilities. We are providing this resource FREE of CHARGE for those who need these required services for placement in one of our 11 skilled nursing facilities.
